For a genetic analysis, a request form and consent in accordance with the Genetic Diagnostics Act are required for the sample material sent in.
Please fill out the request form carefully and completely. Tick the desired tests and add your contact details for any queries. If the registration form is not completed in full, the processing time may be delayed due to queries from our side.
In order to analyse and interpret the test results, we require information about the family history and any previous genetic findings in addition to the clinical details.
In case of doubt, please contact our sample and case management team in advance regarding billing options. If you require a cost estimate (for privately insured persons or billing in an inpatient context), we will be happy to provide you with this in advance.
In accordance with the German Genetic Diagnostics Act (GenDG), the written consent of the person seeking counselling or their legal guardian is required before a genetic test can be carried out. Please enclose a consent form signed by the person seeking counselling or the legal guardian as well as the medical person providing information with the sample.

Please use the right sample containers required for the respective test method. The sample containers must be clearly labelled with the person's first name, surname and date of birth.
